To streamline your visit and ensure we have all necessary medical history, we request that new patients complete their intake information prior to arrival.

Preparing for Your Visit

Please remember to bring the following items to your scheduled appointment:

• Valid government-issued photo ID (Driver's License, Passport).
• Current health insurance card.
• A list of all current medications, including dosages and supplements.
• Relevant medical records, lab results, or imaging (X-rays, MRIs) from your referring physician.
• Preferred pharmacy's name, address, and phone number.

Consultation Fees

While many urological procedures are covered by insurance, all aesthetic and VIP regenerative care services are self-pay. Our front desk will provide full transparent pricing prior to any treatment.
